"The First Lady"

An award-winning documentary film that dares to drive real change for the trans community.

A documentary film by Sagi Bornstein and Udi Nir tells the life story of Efrat Tilma, one of the pioneers of Israel’s trans community. But it is much more than the story of one woman. It is a collective narrative about the trans community, about Israeli society, and about the pain, silencing, and resilience of those who fight simply to exist.

This film is not just a story. It’s a call to action.

Mental health services are a human right, not a privilege

In the film’s hardest moments, and its quiet glimpses of grace, one truth becomes undeniable: we cannot wait any longer. Mental health is a basic need, not a luxury. Yet for many in the trans community, Israel’s mental health system is neither accessible nor safe. This is why we are coming together with a shared goal:

To establish Israel’s first community-based mental health clinic for the trans community.

A clinic led by experienced, trauma-informed community professionals. A clinic that is financially accessible, open to all, and free from exclusionary criteria.
